Three things to read before you order.

Capacity matters. You must have the capacity to make the document at the time you sign it. We assess capacity in conference with you. If we have any concern about capacity, we will tell you so and arrange a medical opinion before drafting proceeds.
You must be the person giving the instructions. A will, power of attorney, or guardianship cannot be ordered by a friend, relative, or carer on someone else's behalf. If you are concerned that a family member needs one of these documents but cannot complete the form, phone the office and we will speak with them directly.
